Lesson Makeup Request Form (Langley)

Welcome to the Lesson makeup request form. Kindly fill out the form below and upon submission you will receive a confirmation email.

– If your lesson is in-person, your date and time selected in the request form will be auto-confirmed, hence no follow-up is required and you may attend the makeup.
– If you would like to set up makeups for multiple children/ siblings, we ask that you please fill out the request form for each child separately.
– If your makeup date falls on one of our major tournament events, our team will contact you to reschedule.
– We are closed on all statutory holidays, hence no makeups will be available during this time.
– You can have a look at our Lesson and Makeup Policy here. Failure to attend the confirmed makeup lesson shall not qualify for a re-schedule.
